 note = {本研究は,NICU看護師の語りから看護者からみたNICUに長期入院している子どもの問題と課題を明らかにする目的で行われた.研究協力者は,NICUに長期入院した子どものケアを行ったことのある看護師15名である.データ収集方法は、非構成面接でNICUに長期入院している子どものケア思い出し,そのときの看護師自身の気持ち,考え,ケア,問題を自由に語ってもらった.分析は、内容によって要素とその関係性を探求した.その結果看護師が語った問題は,それぞれに関連しあっているが子ども,親・家族,看護師,病院・病棟,社会の側面に分類された.看護師は,長期入院している子どもの成長発達にNICUの環境が適していないと感じつつ,また,生命倫理的な問題も複雑に絡み合っており,治療の中止や停止の選択の際に,必ずしも看護師がその選択に納得できないジレンマや強いストレスと直面しながらも,その実践において一生懸命にケアをおこなっている.この間題を日常の看護活動の側面から解決しようとするならば,長期化する前に倫理的な問題の検討,Development Care,子どもの行動の意味を知る, Family Centered Careを徹底させることが不可欠である., The purpose of this research is to investigate the issues that nurses identify with infants requiring long-term care in NICU. The subjects were 16 NICU nurses who were experienced in long-term care of neonates in NICU. Data was collected through interviews using an open-ended questionnaire. Factors and their relationships were explored through content analysis. Then issues were extracted and classified into the six categories: <child>, <parent>, <family>, <nurse>, <hospital> and <society>. NICU nurses involved in long-term intensive care of neonates struggle with such issues, which often causes professional dilemma and a lot of pressure.},
